• Program participants must be at 18 – 65 years of age. • Unity pharmacist will ascertain consumers’ suitability to participate in program. • Only purchases made at Unity are eligible for the program. • Free Starter Kit is only issued upon purchase of first box of OPTIFAST® at Unity. • Free box of OPTIFAST® will only be issued when all 5 boxes of OPTIFAST® are purchased. • Customer must produce the original program card upon payment to collect the stamps. No stamps will be given after payment. • Items are redeemed on a while-stocks-last basis. Items are non-exchangeable, non-refundable and non-replaceable. • Lost program card will not be replaced and is invalid when altered.• Weight loss attained throughout this program differs between individuals. • Closing date of enrollment is 30th June 2021. • Only participants who complete the program by 30th June 2021 can qualify for grand prize • Redemption will be qualified by the Unity Pharmacy Store. • Redemption may change without prior notice due to availability. • Nestle reserves the right to contact program participants for the purpose of gift redemption justification. • By participating in this program, participants agree to be bound by the official rules and regulations and decisions of Nestle Singapore (Pte) Ltd. • Nestle reserves the right to terminate this Program or amend the terms and conditions without prior notice. • 1 winner will be selected by 30 April, 31 May, 30 June 2021 respectively. To qualify for this, completion date for A Healthier U program must be by 30 June 2021. Based on the highest percentage of change from original weight.
